Toffee Topped Rice Pudding

I found and used, with excellent results, a recipe very similar to this one years and years ago; unfortunately I lost it, and wasn't game to try it from memory.  Imagine my surprise and delight when, whilst perusing my much thumbed copy of Elizabeth David's French Country Cooking, I found and adapted this one.
